Information Required

The Management Referral Health Check form requires the following information:

• Information on the employee including date of birth, job title, and date employment commenced
• Information on the sickness including the date the sickness commenced and the diagnosis as it appeared on the doctor's sick note
• A brief outline of the problem
• Details of any work related accident / incident
• Details of any litigation
