Pre-Employment Health Assessment

Information Required

To complete a Pre-Employment Health Check, the employee must provide the following details:

Personal Details
Information such as date of birth, next of kin and name and address of GP must be provided.

Immunisation Status
The employee must indicate whether or not they have ever suffered from any of a list of illnesses or whether or not they have been vaccinated against these illnesses.

Occupational History
The employee must provide information on their employer and job title history.

Medical Questionnaire
The employee completes questions on their current state of health and history including whether they are taking any medication or under the care of a hospital or clinic. The questionnaire also asks for informaton on consumption of alcohol.
